The following symptoms/conditions in the presence of a life-limiting illness or progressive disease may be indicative of a patient's eligiblity for hospice services. A culmination of symptoms, which may not be outlined in this resource, may support clinical eligibility for hospice care. This tool is a guide and a patient may qualify for hospice, even if not all conditions are met. With this fact in mind, it is always recommended the hospice be contacted to evaluate a patient's condition. Determination of hospice eligibilty and prognosis are made by the patient's attending physician and the hospice medical director with input from the hospice interdisciplinary team.
